Alumni Awards Program: Nomination Form

Before submitting a nomination, please be sure to review the award descriptions and criteria (if you have not already done so). For a nomination to receive full consideration, the following form must be completed, including a recommendation statement describing, in some detail, why the nominee is deserving of an award. Self-nominations will not be considered. Alumni can only be nominated for one award each year. The nomination deadline is Friday, August 30th. Recipients will receive their awards at a ceremony Thursday, May 7th, 2020, in New York City.
